During the meeting, Professor Han Fang from the Department of Respiratory and Sleep Medicine of Peking University People's Hospital brought us a new strategy for the management of chronic diseases such as sleep apnea - the application of telemedicine: from theory, technology to the construction of application models. Professor Han said that for the remote management of chronic diseases such as sleep apnea, it is not only necessary to focus on the changes in disease-related AHI indicators, but also to consider the overall control of OSA and various comorbidities of patients, so as to promote the remote management of chronic respiratory diseases in a comprehensive direction.


At present, the difficulties faced by OSA diagnosis and treatment in my country are low awareness, low diagnosis rate, and low standardized treatment rate. There are 1.76 million OSA patients in my country, of which 0.6 million patients know that they have OSA, but the probability of receiving clinical diagnosis and treatment for OSA among the 70 million people is only 16.7%, and only XNUMX% of the XNUMX% of moderate to severe OSA patients choose positive pressure ventilation treatment. Therefore, while improving clinical medical skills, some new strategies, new products, and new technologies must be adopted, especially the need to vigorously carry out home-based, long-term, and remote remote monitoring and treatment management.

Non-invasive positive pressure ventilation (NPPV) is an important respiratory support method in modern times. Since it does not require the establishment of an artificial airway, it is usually used as the first-line treatment for patients with mild to moderate respiratory failure. However, about 50% of patients receiving CPAP (Continuous Positive Airway Pressure) treatment internationally will stop treatment in the first week; in the 2nd to 4th week, the failure rate of patient treatment is 25-50%; among the patients who insist on using it, 12-25% of the population will have the idea of giving up CPAP treatment within three years.


Professor Wang said that in view of the low compliance of PAP (Positive Airway Pressure), we need to continuously improve medical skills, strengthen basic disease education for patients, pay attention to the pressure/flow stability of sleep respiratory therapy equipment, and promote the precision and intelligence of NPPV treatment through a deeper combination of medicine and engineering, so as to achieve technological breakthroughs and innovations based on diseases and patients. He also hopes that in the future, in the closed loop of SDB (Sleep-Disordered Breathing) diagnosis, treatment and follow-up management, there will be more remote diagnosis and treatment technologies to cooperate with clinicians to improve the diagnosis and treatment system with NPPV as the core.
